Borang UD 2A is a critical application form used by medical officers in Malaysia, specifically for Grade UD41 (Contract) Medical Officers who have completed their graduate training (housemanship).

Submission Timeline: You should submit the completed form to the KKM Human Resource Division as soon as training is completed, or at least six (6) months before your current contract expires.

MMC Full Registration Proof: A copy of your notification or official full registration certificate from the Malaysian Medical Council (MMC).

Payment Receipt: A printed receipt of payment from the MMC MERITS system.

Borang 7: Accompanied by a recent passport-sized photograph.
