A mysterious alien lifeform known as the Worms — creatures that can mimic humans perfectly and move at incredible speeds — have been infiltrating society. To fight them, the secret organization ZECT has developed the Masked Rider System, using the Zecters captured from the meteorite.
A new Worm appears in Tokyo, attacking civilians. ZECT dispatches its agents, including Arata Kagami, a young man desperate to become a Kamen Rider. He attempts to use TheBee Zecter, but it refuses to accept him.
Meanwhile, a cocky but skilled young chef named Souji Tendou (now grown) lives with his younger sister, Juka. He hears the commotion and heads toward the battle. Kagami, believing he can still activate a Zecter, reaches for the Kabuto Zecter — but it flies past him and into Tendou's hand.
Tendou calmly says:
"I will rule over everything."
He raises the Zecter, slides it onto the Kabuto Belt (which appeared from nowhere on his waist), and transforms into Kamen Rider Kabuto.
Tendou returns to the restaurant where Hiyori works. He orders his signature coffee (extremely particular about how it’s made). As Hiyori serves him, she notices he is eating alone — at a table set for two.
He quietly says:
"I was waiting for my sister... but she never came."
The implication: His sister was lost in the Worm attack 7 years ago (or was taken by ZECT?).
The episode ends with a shot of Tendou looking up at the sky, repeating his grandmother’s words:

The Worm attempts its signature Clock Up ability — moving at super-speed to evade attacks. Kabuto responds: "Clock Up." He matches the Worm’s speed, and the fight shifts into a frozen-time perspective where only the Riders and Worms can move normally.
Kabuto overwhelms the Worm, then ends it with his finishing move: Rider Kick — a rising, flying kick that destroys the Worm and causes it to explode.
Kagami watches in shock. Tendou detransforms, looks at him, and says nothing, walking away.
The episode opens with a flashback to 1999, when a giant meteor dubbed the "Shooting Star" is on a collision course with Earth. The Japanese government, anticipating impact, scrambles disaster protocols.
Instead of a catastrophic extinction event, the meteor breaks apart in the atmosphere. Large fragments crash into various locations, including a rural riverside. A young Souji Tendou is there with his grandmother. A massive chunk of rock lands nearby, splitting open to reveal a strange metallic object — a silver belt-like device. Inside the meteor fragment, alien worms writhe and begin to emerge.
Tendou’s grandmother takes the belt and hands it to him:
"Grandmother said this... 'Walk the path of Heaven, and rule over everything.'"
